Community Care Services Program

Provides in-home support to help elderly and disabled individuals remain safely in their homes instead of entering a nursing facility.

EDWP (CCSP) assists individuals who are 21 and over and functionally impaired to continue living in their own homes and communities as an alternative to nursing home placement. The provider provides quality services, consistent with the needs of the individual member, which are effective in improving/maintaining the member's independence and safety in the community as long as possible.

Service Options Using Resources in a Community Environment

Combines healthcare and support services to assist individuals with chronic conditions in maintaining independence at home.

VA Geriatrics & Extended Care

The U.S. Department of Veterans Affairs Geriatrics and Extended Care (GEC) program provides a wide range of long-term care services designed to support aging veterans and those with chronic health conditions. The goal of GEC is to help veterans maintain their independence, improve their quality of life, and receive care in the least restrictive setting—preferably in their own homes or communities.These services are tailored to meet the individual needs of each veteran, whether they require short-term assistance or ongoing long-term support.

Respite Care

Respite Care is a program that pays for care for a short time when family caregivers need a break, need to run errands, or need to go out of town for a few days. Respite Care can be helpful to Veterans of all ages, and their caregiver.

Home Respite Care is a service that pays for a person to come to a Veteran's home or for a Veteran to go to an adult day health care program while their family caregiver takes a break or runs errands. The program is for Veterans who need help with activities of daily living. Examples include help with bathing, dressing, or fixing meals. This program is also for Veterans who are isolated, or their caregiver is experiencing burden. Respite Care can help lower the stress the Veterans and their family caregiver may feel when managing a Veteran's short-term or long-term care needs at home.

Veterans Directed Care

Veteran Directed Care gives Veterans of all ages the opportunity to receive the Home and Community Based Services they need in a consumer-directed way.

This program is for Veterans who need personal care services and help with activities of daily living. Examples include help with bathing, dressing, or fixing meals. This program is also for Veterans who are isolated, or their caregiver is experiencing burden.
